What are your requirements for model releases?

Model releases must be legible, in English (or have a side by side translation), contain body copy similar to the standard Veer model release forms, and provide the following information:

- Model's full name (either printed or typed)

- Model's complete contact information

- Model's date of birth

- Date of the shoot and date the form was signed

- Photographer and/or studio name

- Model's signature

If the model is a minor, both the full names of the model and of the model's parent or guardian are required, and the parent or guardian must sign the form in lieu of the model. Other optional information can include the model's ethnicity, a description of the shoot, model's gender, and a reference image of the model.
